1.) Vaccinations:  All dogs must be fully vaccinated. A certificate or proof of vaccinations to verify that your dog has been vaccinated must be presented on or before arrival. Bordetella Bronchiseptica (kennel cough), must have been administered within the last year and at least 7 days before arrival. DAPP (Distemper, Adeno Virus 2, Parvo and Parainfluenza) which is usually administered as a 4 in 1 vaccination, must also be up to date and given at least 7 days before arrival. If vaccinations are due to expire during your dogs stay, they must be updated before arrival. We cannot accept dogs for boarding unless a certificate or proof of vaccination is presented on or before arrival. 

2.) Fees: Fees are charged on a per night basis. Check-out time is 11:00 am. Collections after 11:00 am will be charged as a full day of daycare regardless of what time you checked in.         Please initial ____________ 

3.) Appointments: Delivery and collection of your dog is by appointment only.

4.) Critters: Please ensure that your dog is free from lice/ticks/fleas. If we need to treat your dog we will also need to charge you for this.

5.) Veterinarian authorization: The owner/agent/guardian authorizes Woofy World Kennels & Daycare to engage a veterinarian to treat your dog whenever Woofy World Kennels & Daycare deems it necessary and the owner/agent/guardian guarantees payment of veterinarian fees.

6.) Release of responsibility: We consider our facility and management practices to be of the highest standards, therefore the very best of professional care and expertise will be given to your pet. However, we accept no responsibility for sickness or injury contracted or suffered while your dog is being boarded at Woofy World Kennels & Daycare. We also accept no responsibility for death or loss for any reason whatsoever. The owner/agent/guardian hereby releases Woofy World Kennels & Daycare from all liability for any loss or expense that the owner/agent/guardian may suffer or bear as a result of any happening (such as disappearance, runaway, sickness, injury, death from any cause, fire, theft, etc.) through transportation or otherwise, during the time that you dog is being boarded on or off the premises at 4940 Chute Lake Road, Kelowna, BC.

7.) Un-spayed Females: Our aim is to provide an environment as stress-free as possible for all dogs staying with us. For this reason we will not accept dogs in heat as this unsettles our other guests. A contact person must be given that can collect the dog should this problem arise. A surcharge maybe charged if a dog comes into season during her stay with us. Your dog would immediately be separated from any un-neutered males but Woofy World Kennels & Daycare does not accept any responsibility should your dog become pregnant.

8.) Excessive Barking: Due to noise restrictions, we cannot board any dog that has a problem with excessive barking. Any new dog staying with us will therefore require a contact person who can take the dog if such a problem arises.

9.) Photographs/Videos: Photographs and videos of your dog(s) may be taken during your dogs stay. I authorise Woofy World Kennels & Daycare to use these photographs/videos for advertising and marketing.

10.) Payment: All charges are to be paid when collecting your pet. In long-term situations of two weeks or longer, we request that payment be made monthly or by a 50% deposit on delivering your dog. Balance is to be paid upon collection of your dog.

11.) Abandonment: In the event that the owner/agent/guardian abandons his/her dog while being boarded at Woofy World Kennels & Daycare, or fails or refuses to pay the fees in advance, then after two weeks notice of abandonment or default in fees to the owner/agent/guardian at the address given, Woofy World Kennels & Daycare reserves the right to make any suitable arrangements for the dog.

12.) Expenses: By signing this document, the owner/agent/guardian guarantees that he/she will become personally liable for all expenses for boarding, veterinarian fees (for own dog or injury to another dog caused by your dog), property damage fees, or other expenses incurred while the dog is being boarded at Woofy World Kennels & Daycare.
